<description>&#60;p&#62;I tend not to do a burst but I'm still on 2mg per day.  Just starting to drop to 2mg one day then 1mg the next.  My consultant seems pretty happy with me staying at that level.&#60;/p&#62;
&#60;p&#62;I then take weekly doses of MTX (Methotrexate).  We kind of found a balance of low dose of pred and a low dose of MTX that works.&#60;/p&#62;
&#60;p&#62;Maybe I should work harder at dropping the steroids.

&#60;p&#62;I've been on Enbrel and naproxen for almost two years, since being diagnosed with RA, and feel so lucky that those meds alone have kept things under control.  But a few months ago, my wrists and hands decided to rebel, I guess, and the pain/swelling got so bad that the list of things I literally could not do got so long that I went to my rheumatologist in agony.  She's a good doctor, and prescribed me a prednisone burst of 15 mg for three days, 10 mg for three days, and 5 mg for the final three days.  It was a miracle, and I was able to use those pain-free days to actually accomplish some things - it's amazing how much we take our healthy hands for granted.  &#60;/p&#62;
&#60;p&#62;Anyway, that was my first prednisone burst, and I wasn't sure how often I can use it for relief, if I need to in the future. Every few months if necessary.  Thanks for relating the dangers of corticosteroids - why is it that the medicine that provides almost instant and complete relief is so dangerous?

<description>&#60;p&#62;Yesterday my rheumatologist gave me a lesson in corticosteroid use, in response to my question as to why he didn't have me take it more often when the results always seem so good.&#60;/p&#62;
&#60;p&#62;He pulled out textbooks and rheumatology journals. Every entry on the topic that he shared with me started by highlighting that corticosterioids should be used only for short periods of time, that they should be one of the medicines of last resort, and that the dosage should be as low as possible with tapering commencing almost immediately.&#60;/p&#62;
&#60;p&#62;He explained all of the long-term side-effects associated with corticosterioids, and told me that as with any drug there is always an associated cost and benefit. He told me he takes into consideration severity of RA activity, prior case history and response to previous treatments, age, and gender (primarily due to risk of increased osteoporosis).&#60;/p&#62;
&#60;p&#62;At the moment, it seems that he is prescribing prednisone bursts about every 2 to 3 months. Depending upon how bad my flare is, the bursts lasts two to three weeks and my highest dosage so far has been 10mg/day.
