They always place a big glob of voltaren gel in a joint and then rub it with the ultrasound equipment for 5-7 minutes which is supposed to help the gel penetrate even deeper than usual. I was a little skeptical at first, thinking it was like a ben gay or icy hot, but it wasnt. It took about 15 minutes after applying it to work but it definately relieved the pain. You can only use it 4 times a day on different joints.
